- Minutes from Board Meeting (Excerpt from last board meeting where officers were elected. Specifically, the Secretary of the Corporation, since they will sign the account authorization)

- Assumed Name Certificate (if applicable)1

- Minutes from Board Meeting (Excerpt from last board meeting where officers were elected. Specifically, the Secretary of the Corporation)

- Assumed Name Certificate (if applicable) if the business of the partnership is conducted under an assumed name (a name that does not include the surname of all the partners), then an assumed name certificate should be filed with the county clerk.
